The following information should be for the designated head of household. That person will be the contact for the entire household.

1a. Date of birth

Enter the head of household's date of birth (e.g. 03/2/1975 for March 2, 1975). Do not use commas or spaces.

1b. Mobile phone number

Enter the head of household's mobile phone number, including the area code (e.g. 206-333-1000). Do not use commas or spaces.

1c. Other phone number

Enter another telephone number for the head of household, including the area code (e.g. 206-333-2000). Do not use commas or spaces.

1d. Email address

Enter the head of household's email address, or an email address Seattle Housing Authority can use to contact the head of household.
If you do not have an email address, you can choose a free email provider.
